A dental implant is a titanium or ceramic post

(like a tooth root) that is surgically placed into the jawbone

beneath the gum line. After it is left to heal, the dentist can

then attach a replacement tooth or teeth onto the dental

implant. An implant doesn’t come loose like a denture can;

it acts like a natural tooth root and integrates with your bone.

Dental implants also benefit general oral health because they

do not have to be anchored to other teeth, like bridges.

Philip is a field leader and a Fellow of the International Congress of Oral Implantologists. Philip carries out all aspects of implant dentistry, including single-tooth implants, multiple implant cases, full arch dental implants bone regeneration and bone grafting. He also has a special interest in the field of immediate loading of dental implants.

Having originally graduated from the Dental School at Queen’s University Belfast in 1999, Philip spent a year in general practice before he became the Senior House Officer in the Oral Surgery department at Poole Hospital. There he gained invaluable experience in dentoalveolar surgery before gaining his Diploma of Membership of the Faculty of Dental Surgery (MFDS) in May 2001. Philip has completed many post-graduate courses, notably the one-year restorative course with Michael Wise in 2004. Philip is committed to remaining at the forefront of implant dentistry and keeping up-to-date with advances in the field.

Are dental implants painful?

Although often patients find it surprising, no. We numb the area using anaesthetic, similarly to if we were carrying out a filling. After a few minutes of discomfort for us to numb you up, the procedure itself is pain free. If we lied to you and said we were doing a filling, other than being gowned up to ensure we're sterile, you wouldn't know the difference. Majority of patients do not experience pain after treatment, although some minor discomfort may be experienced, over the counter pain relief is sufficient to reduce this. For more complex cases such as teeth in a day, you may have some swelling and discomfort for 2-3 days after treatment. The majority of patients are very pleasantly surprised how easy dental implant can be, but to be on the safe side we say to expect some minor discomfort and swelling, just in case.

Are dental implants safe?

Yes. The main risks are implant failure or nerve damage. Nerve damage occurs when the nerve is hit when drilling into the bone and damage occurs. This often isn't reversible. We carry out a 3d CT scan of your teeth prior to treatment, which shows exact locations of nerves and your bone levels to the mm to accurately allow us to treatment plan for your bone levels and nerve locations. Implants come in various different widths and lengths to ensure we can place the most suitable implant for you without the risk of nerve damage. Of all our patients who have had dental implants, we are yet to have any experience nerve damage. 

Implant failure is currently 1.08%. Failure is more common in heavy smokers as it can impact the healing process. If your implant we're to fail, we would remove it, allow the site to heal and replace it free of charge. We check your implant has integrated into the bone and healed 

Are dental implants worth it?

It depends on what your desired outcome is for having dental implants. If you have a denture you would like to get rid of to improve your function and no longer require a denture yes. If you having missing teeth you're self conscious of or that impacts your eating and diet yes, if your teeth are failing and you are going to have multiple gaps yes. Some patients however don't mind having a gap, denture or a bridge. Both a denture and bridge will likely need replacing over time, but is a cheaper treatment option initially. Dental implants, once placed and if well maintained (regular check up sand hygiene like normal teeth), should last your life time, so although are more costly, are a more long term investment.

 

Dental implants literally change peoples lives. The amount of patients who have had to learn how to smile again because they have been so self conscious for so many years or patients who haven't eaten an apple or steak for as long as they can remember, these are the patients dental implants are hugely beneficial for. Even single teeth implants are a good option as a bridge involves filing the teeth either side or a maryland (butterfly) bridge that doesn't file the teeth either side but is glued in and can have a tendency to come out. You can not treat a gap of course but that can result in other teeth being over loading and reducing their long term prognosis.

Can dental implants be removed?

You would not remove a dental implant unless essential, which is very unusual unless it is failing. Once integrated into the bone in the initial few months of being placed, it is then usually there for life. The crown/bridge/denture on top can be removed. A denture can be removed by the patient at home for cleaning and clipped back in. A bridge and crown and only be removed by your dental professional and has no reason to be removed unless there was a complication. It is designed to be fixed long term.

How do dental implants work?

Think of a dental implant as a raw plug that is screwed into the bone, the bone then grows into the thread of the implant holding it in place. An abutment is then screwed into the implant that the crown or bridge is glued or screw retained onto. If you have a denture it clips into place similarly to a very strong popper. Crowns and bridges are permanent fixed options, a denture is removable, although has significantly more retention than a standard denture, no denture fix would be required.

How long do dental implants last?

They should last you lifetime pending correct care. You will still need to brush your teeth and implants daily as if they were normal teeth. You will require regular check ups and hygiene appointments to ensure good gum health to aid longevity of your implants. We do offer a life time guarantee for our dental implants. Your life time can impact your implants, for example heavy smokers have a higher failure rate than no smokers.

What are dental implants made of?

Titanium, we do have a ceramic alternative available if the patient prefers, but as standard titanium is used.

What dental implant is best?

This is a tricky question. Every brand is slightly different but they are all generally quite similar. We use Neodent and have done for over 7 years, Phil, our Implantologist, has placed over 3000 implants with Neodent, and has currently placed the most in the UK. He has over a 98% success rate with the system. It is more recommended to choose the right dental implantologist for you, rather than the system, they will have a system they are experienced with and confident in. 

Why are dental implants expensive?

Dental implants are more costly due to the surgical procedure required, the time in surgery and the longevity of the treatment. You generally pay more for implants than a bridge for example, however a bridge will need replacing multiple times in your life time, implants don't. If you're happy to pay a cheaper treatment fee but accept you will need to have repeat treatment a bridge or denture is a good option, if you prefer treatment once and not need to repeat it, it is advised you opt for dental implants, pay more initially but don't require repeat treatment long term.

What's the difference between an implant retained crown, bridge and denture?

An implant retained denture is a denture that is removable, the same as a standard denture, however it clips into your implants so you get a lot better fit and retention than a standard denture so denture fix isn't required. It is still a removable option. 

 

An implant retained crown is glued onto an abutment (screw) that goes into the implant to replace a single tooth. It is not removable by the patients.

 

An implant retained bridge replaces multiple missing teeth and is screwed into your implants. Your dentist can remove the bridge if required, but it is a fixed replacement of multiple teeth for patients who don't want removable teeth. Implant retained bridge and crowns are more common than implant retained dentures. Implant retained dentures are cheaper for full mouth replacement but  don't give you as good function as a implant retained bridge will.
